Liability Waiver

Everyone 18 years of age & older that wants to skate MUST SIGN this Hold Harmless Contract. I waive all responsibility to Classic Skating and Fun Center Orem at 250 State St. Orem UT 84058, Classic Fun Center Sandy at 9151 S 255 W Sandy UT 84070, and the Houston Rolling Committee, on these dates 09/19/2025 - 09/20/2025. I am 18 years of age or older and assume responsibility for my actions while skating. I will use my adult knowledge to make wise and safe decisions and realize that skating is a sport and that accidents may occur. If choosing to skate I acknowledge the risks I am taking while skating and take full responsibility for any injury that I may cause to myself or another skater. I am choosing to skate at my own risk. I have read over and understand my responsibility while choosing to skate and attend the entire 4th Annual Salt Lake City Retreat Weekend: Fri, 09/19/2025 - Sun 09/20/2025.
